Question:
I want to have a child in the near future, and I am not sexually active because I do not have a committed partner. ... I might have to look for a sperm donor to have a child.
My periods are normal and right now, no pains at all. I had chlamydia over 14 years ago from my husband, and the Planned Parenthood doctor forgot to treat me for it until a year later and said oops.
How do I know if I'm sterile or not? What are the symptoms?
Answer:
This is an excellent question, and one that we hear not infrequently. Since female fertility has two basic components -- ovulation/hormonal support and fallopian tube patency -- both need to be considered in your case.
If you are less than 35, your general fertility has not declined substantially. To assess for ovulation, you can do basal body temperatures for several cycles to document ovulation. This would be important if you are planning donor insemination. You could also do LH surge prediction kits (found in the drugstore) to confirm probable ovulation function.
Given your gynecologic history of a single episode of chlamydia, my bigger concern would be tubal scarring (preventing the egg/ovum from reaching the uterus). The most reliable way to assess this is an HSG or hystersalpingogram. Dye is injected through the cervical canal into the uterus and then out the tubes. A type of X-ray (fluoroscopy) takes real-time pictures to observe if the dye is freely going out into the pelvic cavity. This would suggest that both tubes are indeed open. HSG is an expensive and uncomfortable test. Generally this is done if the woman is unable to get pregnant after trying for 6-12 months. Since you would be doing donor, your situation may be different. One would be less likely to do repeated donor attempts for 6-12 times.
Since you will be seeing a reproductive endocrine MD or infertility MD (unless you are doing "turkey baster" from a male friend) you should elicit their recommendations on the chlamydia/tubal issue. You could ask our WebMD infertility specialist on the Trying to Conceive message board for an opinion as well.
I wish there were an easy answer to this. Often, a woman may be told "go ahead and try initially before an HSG is done." And we have ALL had patients who were told "Your chance of pregnancy is small to none" and then the woman got an unplanned (but welcome) pregnancy!
