Dong Quai - Latin binomial: Angelica sinensis

Synonyms: Tang Kuei, Dang Gui, Female Ginseng, Female Tonic, Woman's Herb, Toki, Tangwi

Part used: Root



Constituents:
Volatile and fixed oils notably ligustilide, beta-sitosterol, palmitic acid and safrole. Furanocoumarins (psoralens and bergapen), Vitamins A, E and B 1 2, pantothenic acid, folic acid, biotin. Organic acids: ferulic acid, succinic acid, myristic acid.1-4

Emmenagogue, Sedative, Analgesic, Blood Tonic 1

Pharmacology
Recently, Dong Quai has received much attention for its action as a uterine tonic. It exhibits a biphasic action directly upon the uterus. The degree of the intrauterine pressure defines the extent to which Dong Quai's components exert their actions. When it is low, there is inhibition of uterine muscle contraction and increased blood flow into the area. As the pressure increases, the arrhythmic contractions become rhythmical and slow, giving the muscle a chance to rest. The overall action exhibited therefore normalizes uterine tonicity. The exact mechanism is as yet unknown, but does not appear to be estrogen mediated.2-4 Dong Quai has cardiovascular properties, having been shown 'in vitro' to inhibit cardiac muscle contraction and exert a hypotensive action. The influence on blood pressure could in part be due to peripheral relaxation of smooth muscle, mediated by an action on muscarinic and histaminic receptors. It also exerts a similar action on gastrointestinal smooth muscle, resulting in a mild laxative effect.2-5 This herb possesses sedative, analgesic, hepatoprotective and antimicrobial properties. The antimicrobial action has been shown to be directed against a wide range of bacteria including hemolytic Streptococcus, but appears to be weak and inconsistent amongst related species.2,4,6

Indications
Used in Traditional Chinese Medicine as a 'blood tonic' Commonly used to treat menstrual problems such as dysmenorrhoea, amenorrhoea, irregular menstruation and metrorrhagia as well as menopausal changes Used orally or parenterally for pain, bruising and trauma 1-7

It is receiving renewed interest in the treatment of cardiovascular disease, especially angina and coronary heart disease.2-5

Drug Interactions
Dong Quai increases the absorption of Vitamin E.5,6

Toxicity and Contraindications
Though unlikely, occasional diarrhea may occur when taking Dong Quai. The furocoumarins may lead to photosensitization resulting in a dermatitis-like skin reaction.8 Due to its effect on uterine muscle, it is contraindicated in pregnancy. It should be noted that Dong Quai has received a great deal of attention for the treatment of menstrual complaints. While it is useful in this regard, it should not be considered appropriate in every instance.

Administration
Historically, Dong Quai was taken as a tea, decoction or part of a soup. It is available in tablets, capsules and hydroalcoholic tinctures.

Dosage:
3 to 10 gm of raw herb or equivalent daily.3

It is often combined with other herbs specific to menstrual problems: Dioscorea villosa (Wild Yam), Vitex Agnus Castus (Chastetree), Cimicifuga racemosa (Black Cohosh), Caulophyllum thalictroides (Blue Cohosh).
